29 CFR 1926.454(a): The employer did not have each employee who performed work on a scaffold be trained by a person qualified in the subject matter to recognize the hazards associated with the type of scaffold being used, and to understand the procedures to control or minimize those hazards. On or about May 24, 2024, on the side of the new home under construction; employees were exposed to fall hazard while working on a job-made scaffold without training.

29 CFR 1904.39(a)(2): Within twenty-four (24) hours after the in-patient hospitalization of one or more employees or an employee's amputation or an employee's loss of an eye, as a result of a work-related incident, the employer did not report the in-patient hospitalization, amputation, or loss of an eye to OSHA. On May 24, 2024, an employee was injured while working and had to be life flighted to area trauma hospital where he was admitted. The hospitalization was not reported within 24 hours to the local Area Office of the Occupational Safety and Health Administration.
