OSH ACT of 1970 Section (5)(a)(1): The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees were exposed to struck by hazards from moving vehicles: (a) On or about May 29, 2024, at the receiving dock, employees performing Blood Pad Associate duties including, but not limited to cleaning and guiding dump trucks with attached trailer were exposed to being struck by the moving trucks.

The employer did not ensure that when wet processes are used, drainage is be maintained and, to the extent feasible, dry standing places, such as false floors, platforms, and mats are provided: (a) On or about May 29, 2024, employees engaged in activities such as but not limited to cleaning and assisting truck drivers entering blood pads were working on floors that were not maintained in a dry condition.
