On or about: June 4, 2024 Location: 629 Courtlandt Ave., Bronx, NY 10451 Roof of new gym extension.  a) Two (2) workers utilizing lifeline system that was not rigged to limit the free fall to not more than 6 feet. The lifelines were tied to an I-beam in the roof holes area and the length of the lifeline would allow an employee to fall more than 6 feet through multiple holes in the area.

On or about: June 4, 2024 Location: 629 Courtlandt Ave., Bronx, NY 10451 Roof of new gym extension.  a) One (1) worker was not provided with a personal fall arrest system (PFAS) and walked and worked on the corrugated roof deck and was exposed to unprotected floor hole, thereby exposing him to approximately 20-25-foot fall hazard.
