OSH ACT of 1970 Section (5)(a)(1): The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm in that employees were exposed to fall hazards when standing on the forks of a rough terrain forklift:   On or about June 19, 2024, employees were exposed to fall hazards while cleaning out a Grout Hog GPHC75 from the forks of an Xtreme Model XR360 rough terrain forklift without fall protection.
