29 CFR 1910.303(b)(7)(i): Unused openings in boxes, raceways, auxiliary gutters, cabinets, equipment cases, or housings shall be effectively closed to afford protection substantially equivalent to the wall of the equipment.  a) On or about June 26, 2024, the employer did not ensure unused openings in the Cutler-Hammer UNITROL Motor Control Center (Serial Number - 6AFI386853-A), located in the CRANE MCC room, were effectively closed exposing employees to a 440-volt electrical shock hazard when locking out skimmer buckets, pulling fuses, switching disconnects and maintaining the crane and pit skimmers.

Electric equipment may not be used unless the following markings have been placed on the equipment:   29 CFR 1910.303(e)(1)(i): The manufacturer's name, trademark, or other descriptive marking by which the organization responsible for the product may be identified;  a) On or about June 26, 2024, the employer did not ensure the manufacture's name, trademark, or other descriptive markings were on the electrical disconnects for the LIGHTNIN mixers and lights located in the lime room and hanging on the light pole outside the electrical room, exposing maintenance electricians to electrical shocks hazards during preventative maintenance and repairs.

Distances shall be measured from the live parts if they are exposed or from the enclosure front or opening if they are enclosed;  a) On or about June 26, 2024, the employer did not ensure Maintenance Electricians (MTE) conducting preventative maintenance on live parts of the square D safety switch in the Millwright Room had a minimum depth of three-foot (3') working space (Table S-1, 151-600-volts and condition A) exposing them to an electrical shock hazard.

On or about June 26, 20241, the employer did not ensure the electrical disconnects on the light pole outside the electrical shop were guarded to prevent damage from tractor trailers, PIVs, caustic trucks, personal vehicles, and aerial lifts exposing the drivers of these vehicles to electrical shock hazards.
