29 CFR  1926.1060(a)(1)(iii): The proper construction, use, placement, and care in handling of all stairways and ladders;   On or about June 26, 2024, at 6241 North 157th Avenue Circle, Omaha, Nebraska. The employer failed to ensure the ladder used by employee(s) was setup and placed correctly required by this sub-part. Employee(s) were using the ladder as a plank/gangway to travel from one roof surface to the other exposed employee(s) to a fall hazard greater than 6 feet.

29 CFR  1926.501(b)(13):"Residential construction." Each employee engaged in residential construction activities 6 feet (1.8 m) or more above lower levels shall be protected by guardrail systems, safety net system, or personal fall arrest system.   On or about June 26, 2024, at 6241 North 157th Avenue Circle, Omaha, Nebraska. Employee(s) were working at a height greater than 10 feet from the lower level without using any form of fall protection as required by this sub-part.  Roberto Nicholas Diego Roofing was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1926.501(b)(13), which was contained in OSHA inspection number 1667286, citation number 1, item number 1 and was affirmed as a final order on June 28, 2023, with respect to a workplace located at 4543 South 203rd Avenue, Omaha, Nebraska.
