ABC Technologies WMG USA, Inc. - Bellevue, Ohio: On or about June 27, 2024, the employer did not ensure daily functional tests of the east Engineered Material Handling 50/20-ton overhead gantry crane (model ZLK; s/n 101677) were conducted. Employees were exposed to struck-by and crushing injuries in that daily checks of functions such as, but not limited to, travel and hoist brakes, limit switches, and for wire rope damage, were not performed. b. ABC Technologies WMG USA, Inc. - Bellevue, Ohio: On or about June 27, 2024, the employer did not ensure daily functional tests of the west Engineered Material Handling 50/20-ton overhead gantry crane (s/n 975386) were conducted. ABC Technologies WMG USA, Inc. - Bellevue, Ohio: On or about June 27, 2024, the employer did not ensure that the running wire rope/cable associated with the 20-ton hoist on the east Engineered Material Handling overhead gantry crane was thoroughly inspected at least once a month and inspections were documented. The wire rope was used to lift equipment such as, but not limited to, plastic injection molds; employees were exposed to struck-by and crushing injuries in that the wire rope failed when used to lift a mold that weighed 29,150lbs. b. ABC Technologies WMG USA, Inc. - Bellevue, Ohio: On or about June 27, 2024, the employer did not ensure that the running wire rope/cable associated with the 20-ton hoist at the west Engineered Material Handling overhead gantry crane was thoroughly inspected at least once a month and inspections were documented. Employees who used the crane's hoist to lift equipment such as, but not limited to, plastic injection molds were exposed to struck-by and crushing injuries. ABC Technologies WMG USA, Inc. - Bellevue, Ohio: On or about June 27, 2024, the employer did not ensure that plastic injection machine molds being moved with the east Engineered Material Handling overhead gantry crane did not contact obstructions. Plastic injection molding machine equipment located under the east overhead crane reduced the space for handling the load with the crane, resulting in the load contacting the crane's girders when hoisting. Employees were exposed to struck-by and crushing injuries in that contact with obstructions during hoisting compromises the integrity of crane components. b. ABC Technologies WMG USA, Inc. - Bellevue, Ohio: On or about June 27, 2024, the employer did not ensure that plastic injection machine molds being moved with the west Engineered Material Handling overhead gantry crane did not contact obstructions.
