29 CFR 1926.102(a)(1): The employer shall ensure that each affected employee uses appropriate eye or face protection when exposed to eye or face hazards from flying particles, molten metal, liquid chemicals, acids or caustic liquids, chemical gases or vapors, or potentially injurious light radiation. (a) On or about June 25 2024, at the job site located at 2605 E. Emily Ave, Oak Creek, WI 53154, employees were performing roofing work with pneumatic nail guns without the use of eye protection, exposing them to possible danger of eye injury from flying objects.

29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b): (a) On or about June 25, 2024, at the job site located at 2605 E. Emily Ave, Oak Creek, WI 53154, employees were installing shingles on the residential roof with a 8:12 pitch, were not using fall protection that they were wearing. This exposed employees to a fall hazard of approximately 8 1/10 feet from roof to the ground below.
