On or about July 3, 2024, at a worksite located at 249 North Lake Street in Neenah, Wisconsin: employees cleaning up concrete debris were exposed to an approximately 15 feet fall hazard while walking up to an approximately 100 feet wide by 26 feet deep opening without wearing any form of fall protection.  c. On or about July 3, 2024, at a worksite located at 249 North Lake Street in Neenah, Wisconsin: employees demolishing a concrete floor with a hand chipper and cleaning up concrete debris were exposed to a fall in excess of six feet while working around an approximately four feet wide by 20 feet deep opening without wearing any form of fall protection.
