29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b):   (a) On July 8, 2024, at 303 1/2 Collins Ave in Mandan North Dakota, an employee engaged in residential roofing activities with roof shingles was wearing a harness but was not attached to a lifeline/lanyard at heights greater than 6 feet.

29 CFR 1926.502(d)(15): Anchorage used for attachment for personal fall arrest equipment was not capable of supporting at least 5,000 pounds (22.2 kN) per employee attached nor was it designed, installed and used as follows: As part of a complete personal fall arrest system which maintains a safety factor of at least two; and under the supervision of a qualified person.  On July 8, 2024, at 303 1/2 Collins Ave in Mandan, North Dakota, two individuals working on a roof at heights over 6 feet used two peak anchors on the roof that were not properly secured to the peak in accordance the manufacturer's instructions.

29 CFR 1926.502(d)(17): The attachment point of the body harness was not located in the center of the wearer's back near shoulder level, or above the wearer's head:  (a) On July 8, 2024, at 303 1/2 Collins Avenue in Mandan, North Dakota, one individual working at heights over 6 feet was fastening roofing materials. The individual was wearing a harness and had a rope tied to the webbing on the side of the harness.

29 CFR 1926.416(e)(1): Worn or frayed electric cords or cables were used:  (a) On July 8, 2024, at 303 1/2 Collins Ave in Mandan, North Dakota, employees were exposed to electrical hazards where the protective outer coating of a 120 Volt electrical cord on an air compressor had been worn away exposing electrical strands.
