Section (5)(a)(1): The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to struck-by hazards:  (a) On July 9, 2024, and times prior, at 200 Knutson St, Mapleton, ND, employees were exposed to struck-by hazards while working in close proximity to industrial storage racks that were not secured or anchored against toppling.  Note: Among other methods, feasible and acceptable abatement methods to correct this hazard include but are not limited to:  1. Follow the recommendations of ANSI MH 16.1 - Specification for the Design, Testing and Utilization of Industrial Steel Storage Racks, Sections 1.4 - Integrity of Rack Installations 2. Follow the Interlake Mecalux product manufacturer's support guide under the Anchor section.

Section (5)(a)(1): The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to crushing hazards:  (a) On July 9, 2024, and times prior, at 200 Knutson St, Mapleton, ND, employees working with or in close proximity to equipment elevated by temporary supports/stands were exposed to the hazard of being crushed by falling equipment where support/auxiliary/jack stands were not proof tested.  (b) On July 9, 2024, and times prior, at 200 Knutson St, Mapleton, ND, employees working with or in close proximity to equipment elevated by temporary supports/stands were exposed to the hazard of being crushed by falling equipment where the support/auxiliary/jack stands did not have identification or safety markings.  Note: Among other methods, feasible and acceptable abatement methods to correct this hazard include but are not limited to:  1. Follow the recommendations of ASME PASE -2014 - Safety Standard for Portable Automotive Service Equipment, Sections 2.3 - Product Marking and Identification 2. Follow the recommendations of ASME PASE -2014 - Safety Standard for Portable Automotive Service Equipment, Sections 7-4 and 13-4 Design qualification testing Proof test the stands and label with rated capacity.
