It shall be the responsibility of the employer to maintain all forge shop equipment in a condition which will insure continued safe operation:  On or about July 16, 2024 at 970 East 64th street, Cleveland, OH 44103, the employer failed to perform inspections and maintenance on the 1500 ton precision forge in that the forge press oscillated during normal operations while employees attempted to insert a die block. The company failed to ensure that the valve used to control the hydraulic fluid was properly inspected and maintained in a serviceable condition.
