29 CFR 1910.29(f)(4): Openings in stair rails. The employer did not ensure that no opening in a stair rail system exceeds 19 inches (48 cm) at its least dimension. On or about July 31, 2024, the eastside stairway leading up to the rig platform, employees engaged in oil and gas operations utilized the stairway which had missing mid-rails on both sides while using it to gain access to the rig floor.
