29 CFR  1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system:  a)  On or about August 7, 2024, at the worksite, 610 North Madison Street, Rome, NY 13440: employees engaged in residential roofing replacement were not protected from falling by use of guardrail systems, safety net system, or personal fall arrest systems.  Employee were exposed to fall hazards of 22 feet.  Home Evolution Roofing was the controlling employer.  b)  On or about August 7, 2024, at the worksite, 610 North Madison Street, Rome, NY 13440: employees engaged in residential roofing replacement were not protected from falling by use of guardrail systems, safety net system, or personal fall arrest systems.  Employee were exposed to fall hazards of 15 feet.  Home Evolution Roofing was the controlling employer.

29 CFR  1926.1053(b)(1): Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed:  a)  On or about August 7, 2024, at the worksite, 610 North Madison Street, Rome, NY 13440:  Employees engaged in residential roofing activities were using a portable extension ladder to access the roof.  The employer failed to ensure the side rails of the ladder were extended 3 feet above the access point to the roof.
