29 CFR  1910.178(l)(4)(iii):An evaluation of each powered industrial truck operator's performance was not being conducted at least once every three years:  On or about August 12, 2024, the employer failed to ensure that an employee that operates powered industrial trucks (PITs), such as, the Toyota Electric Forklift Truck (model 8BPUE15) was evaluated on PIT performance at least once every three years. As a result, employee(s) working in the facility, and other forklift operators were exposed to struck-by and caught-in hazards.
