29 CFR  1926.503(b)(1): The employer did not verify compliance with paragraph (a) of this section by preparing a written (training) certification record including the name or other identity of the employee trained, the date(s) of the training, and the signature of the person who conducted the training or the signature of the employer.   On or about August 20, 2024, at the front of the building, employees on the roofs were exposed to falls greater than 6 feet to a lower level, and there was no written certification of fall protection training.

29 CFR 1926.760(a)(1): Each employee engaged in a steel erection activity who is on a walking/working surface with an unprotected side or edge more than 15 feet (4.6 m) above a lower level was not protected from fall hazards by guardrail systems, safety net systems, personal fall arrest systems, positioning device systems, or fall restraint systems.  a.  On or about August 20, 2024, at the front of the building, employees on the roof peak were not in use of fall protection and were more than 30 feet above a lower level during steel erection work that included installation of exterior metal siding.    b.  On or about August 20, 2024, at the front of the building, an employee on the roof was more than 15 feet above a lower level and did not wear a personal fall protection harness and lanyard when working at the sides of the roof during steel erection activities including installation of exterior metal siding.

29 CFR 1926.453(b)(2)(v): A body belt with lanyard attached to the boom or basket was not worn by employee(s) working from an aerial lift    On or about August 20,2024, at the front of the building, employees did not attach a fall protection lanyard to the boom or basket of the aerial lift when they used the aerial lift to access and get down from the building roof which was greater than 6 feet above a lower level.  JP Steel Building Construction LLC was previously cited for a violation of this Occupational Safety and Health standard 29 CFR 1926.453(b)(2)(v), which was contained in OSHA inspection number 1659573, citation number 01, item number 01 and was affirmed as a final order on May 1, 2023, with respect to a workplace located at 2323 N. Harrison Ave., Shawnee OK 74804.
