29 CFR 1926.416(a)(1): The employer did not prevent employee(s) from working in proximity to an electric power circuit that the employee could contact during the course of work and the employee(s) were not protected against electric shock by de-energizing the circuit and grounding it or by guarding it effectively by insulation or other means:  a) On or about 08/08/2024, at the site located at 1119 Euclid Ave, Syracuse, NY 13212: Workers were not protected from electrical shock when using multiple tools, including but not limited to pneumatic nail guns, hammers and scrapers approximatley 1-2 feet from the incoming energized electrical service.

29 CFR  1926.1051(a): Stairways or ladders were not provided at all personnel points of access where there was a break in elevation of 19 inches (48 cm) or more, and no ramp, runway, sloped embankment, or personnel hoist was provided.   a) On or about 08/08/2024, on the site located at 1119 Euclid Ave, Syracuse, NY : Employees were not provided stairways or ladders to access the second story of the residential home and instead were utilizing the RGC Hoist Platform as a ladder.
