29 CFR  1910.147(c)(1): The employer did not establish a program consisting of an energy control procedure, employee training and periodic inspections to ensure that before any employee performed any servicing or maintenance on a machine or equipment where the unexpected energizing, startup or release of stored energy could occur and cause injury, the machine or equipment shall be isolated from the energy source and rendered inoperative:  a) On or about July 25, 2024, the employer failed to protect the employee from unexpected release of hazardous energy during service and maintenance activities on the Sparks Sachet Machine (No serial or identification placard) . The employees were exposed to machine hazards associated with pinch points, burn hazards, and amputation hazards from the unexpected start-up and energization of the heat seal jaws while performing maintenance such as (but not limited to) removing defective product from the heat seal jaws.  Abatement documentation is required for this item in accordance with 29 CFR 1903.19(d)
