29 CFR 1926.453(b)(2)(v): A body belt was not worn and/or a lanyard was not attached to the boom or basket when working from an aerial lift: Note:  As of January 1, 1998, subpart M of this part (1926.502(d)) provides that body belts are not acceptable as part of a personal fall arrest system.  The use of a body belt in a tethering system or in a restraint system is acceptable and is regulated:   On or about August 27, 2024, 02 employees were exposed to a fall hazard from approximately 11 feet above the gravel covered ground while installing siding from inside the basket of an aerial lift without wearing a body belt or PFAS on jobsite located at 210 Northland Drive, MEDINA, OH 44256.

29 CFR 1926.503(a)(1): The employer did not provide a training program for each employee potentially exposed to fall hazards to enable each employee to recognize the hazards of falling and the procedures to be followed in order to minimize these hazards:  On or about August 27, 2024, the employer did not provide a training program for employees to recognize the hazards of falling and the procedures to minimize those hazards.  There were 02 crew members in an aerial lift approximately 11 feet above the ground not utilizing or wearing any form of fall protection while installing siding at a jobsite located at 210 Northland Drive, MEDINA, OH 44256.
