29 CFR 1926.451(e)(1): When scaffold platforms were more than 2 feet (0.6 m) above or below a point of access, portable ladders, hook-on ladders, attachable ladders, stair towers (scaffold stairways/towers), stairway-type ladders (such as ladder stands), ramps, walkways, integral prefabricated scaffold access, or direct access from another scaffold, structure, personnel hoist, or similar surface were not used and cross braces were used as a means of access:   a) 421 Meadowlands Drive, Washington, PA: On or about September 5, 2024, during masonry work, the employer did not ensure that a safe means of getting down from the scaffold's top most level was provided in that the ladder in use was too short to reach this level, resulting in employees climbing cross bracing or walk through type scaffold end frames to get to the scaffold level where the ladder was located.

29 CFR 1926.451(g)(4)(i): Guardrail systems were not installed along all open sides and ends of platforms:   Instances including, but not limited to, the following:  a) Job site, 421 Meadowlands Drive, Washington, PA: On or about September 5, 2024, during masonry work, the employer did not ensure that guardrail systems were installed along the west end of a scaffold foot hop, exposing an employee to a fall hazard of up to at least approximately 20 feet.   b) Job site, 421 Meadowlands Drive, Washington, PA: On or about September 5, 2024, during masonry work, the employer did not ensure that guardrail systems were installed along the east end of a scaffold foot hop, in that an outrigger bracket was used for fall protection,  exposing an employee to a fall hazard of up to at least approximately 15 feet.   c) Job site, 421 Meadowlands Drive, Washington, PA: On or about September 5, 2024, during masonry work, the employer did not ensure that guardrail systems were installed along the mud tub area of the scaffold,
