On or about September 3, 2024, on the east side of the warehouse, an employee reached by hand and did not use a tool to place a piece of expanded metal into the point of operation of a roller leveler machine.

On or about September 3, 2024 on the eastside of the warehouse, at the roller leveler machine operated by employees, a cover was not installed on a unused opening of a conduit fitting.  b. On or about September 3, 2024 on the eastside of the warehouse, at the roller leveler machine operated by employees, a cover was not installed at an unused opening of a duplex outlet box.

On or about September 3, 2024, on the eastside of the warehouse, at the roller leveler machine used by employees, a face plate was missing on a switch box.
