29 CFR 1904.30(a): The employer did not keep a separate OSHA 300 Log of Work-Related Injuries and Illnesses for each establishment that is expected to be in operation for one year or longer: a) 101 E. Main Street NORFOLK, VA 23510: On September 9, 2024, the employer had five (5) establishments in the local area with 10 or more employees at each. Each of those establishments are fully operational and have been for one year or longer. The employer did not keep separate OSHA 300 Logs for separate establishments. Notice 2, Item 1, 29 CFR 1904.30(a) as required by 29 CFR 1960. (8): The employer failed keep a separate OSHA 300 Log of Work-Related Injuries and Illnesses for each establishment that is expected to be in operation for one year or longer. The employer was cited for a violation of the same standard and/or same hazard on November 19, 2021, Inspection #1564816, April 08, 2022, of final order/settlement and means of abatement. Abatement certification and documentation required within 10 days after abatement date. The certification shall include a statement that abatement is complete, the date and method of abatement, and state that employees and their representatives were informed of this abatement. Abatement documentation shall include documents demonstrating that abatement is complete, such as evidence of the purchase or repair of equipment, photographic or video evidence of abatement or other written records
