29 CFR 1910.119(e)(3)(v): The process hazard analysis did not address facility siting for the three ash collection silos.  On or about May 6, 2024, and at times prior thereto, the employer's Process Hazard Analysis (PHA) did not consider facility siting for the three ash collection silos, which were located within the proximity of the facility Ammonia (NH3) High Pressure Receiver. When one of the three silos collapsed, the debris field reached the receiver, engulfing the receiver and the surrounding area, limiting access to the receiver and its components.
