29 CFR 1910.119(j)(5): The employer did not correct deficiencies in equipment that were outside acceptable limits before further use in that:   a) On or about September 19, 2024, in the bleed gas membrane unit at the Renewal Energy Group (REG) Geismar LLC plant, the employer failed to correct deficiencies in the process piping system for LI 5411 exposing employees to hazards from loss of containment of high pressure, high temperature hydrocarbon/hydrogen mixture and the resultant fire and explosion hazard.
