29 CFR  1926.102(a)(1): The employer did not ensure employees used eye and face protection equipment when machines or operations presented potential eye or face injury from physical, chemical, or radiation agents.  (a) On or about October 1, 2024, at 10710 S. Nicholson Rd, Oak Creek, WI 53154, the employer did not ensure employees conducting installation of a roof on a residential home, using hand tools and nail guns were wearing eye protection. This exposed the employees to struck by hazard and eye injury.

29 CFR  1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b).  (a) On or about October 1, 2024, at 10710 S. Nicholson Rd, Oak Creek, WI 53154, employees installing a roof on an existing residential home with an approximately 11:12 pitch were not using fall protection. This exposed employees to a fall hazard of approximately 17 feet 11inches from the roof eve to the ground below on the right (Southwest) side of the home.
