29 CFR 1926.451(c)(2)(v): The employer used forklifts to support scaffold platforms without ensuring that the entire platform was attached to the fork and that the forklift was not moved horizontally while the platform was occupied: Employee(s) were exposed to fall hazards. This was observed on or about October 1, 2024, at 21150 Hartman Ave, Elkhorn, Nebraska 68022. Employees conducting framing activities were exposed a fall hazard in that a supported scaffold platform that was being used with the Lull powered industrial truck was not secured to the forks.

29 CFR 1926.451(g)(1): Employees on scaffolds more than 10 feet (3.1 m) above a lower level were not protected from falling to that lower level by fall protection established in paragraphs (g)(1)(i)-(vii) of this section: Employee(s) were exposed to fall hazards. This was observed on or about October 1, 2024, at 21150 Hartman Ave, Elkhorn, Nebraska 68022. Employees conducting framing activities were exposed a fall hazard in that employees did not have fall protection while working on a Lull mobile scaffold that was approximately 20 feet above the ground.
