29 CFR  1926.451(e)(1): When scaffold platforms were more than 2 feet (0.6 m) above or below a point of access, portable ladders, hook-on ladders, attachable ladders, stair towers (scaffold stairways/towers), stairway-type ladders (such as ladder stands), ramps, walkways, integral prefabricated scaffold access, or direct access from other scaffold, structure, personnel hoist, or similar surface was not used.    a. At a temporally site located at 1905 Promenade Way, Jacksonville, FL 32207: On or about September 30, 2024, the employer exposed employees to fall hazards, in that ladders, ramps, walkways, or similar equipment was not provided to employees to access a scaffold platform that was more than 2 feet (0.6 m) above or below a point of access.

At a temporally site located at 1905 Promenade Way, Jacksonville, FL 32207: On or about September 30, 2024, the employer exposed employees to fall hazards, in that guardrail systems were not installed along all open sides and ends of platforms prior to, and while, employees performed work while supported by that that scaffold system.
