Note: The employer is required to submit abatement certification for this item in accordance with 29 CFR 1903.19.

The employee was accessing the scaffolding plank by using an A-Frame ladder that did not extend at least 3 feet above the upper landing; on or about 9/27/2024. Note: The employer is required to submit abatement certification for this item in accordance with 29 CFR 1903.19.
