29 CFR  1926.102(a)(1): The employer did not ensure that each affected employee used appropriate eye or face protection when exposed to eye or face hazards from flying particles, molten metal, liquid chemicals, acids or caustic liquids, chemical gases or vapors, or potentially injurious light radiation.  On or about 10/7/2024 during the installation of roofing material at a job site located at 5155 Tuscarawas ST W., Canton Ohio 44708; the employer did not ensure employees working on the roof of a commercial facility were protected from possible danger of eye injury from impact, or from flying objects from the employees preparing the roofing surface (using a pressure washer) for the application of waterproofing material.

29 CFR  1926.502(f)(1):Warning lines were not erected around all sides of the roof work area:  On or about October 7, 2024, during the installation of roofing material at a job site located at 5155 Tuscarawas ST W., Canton Ohio 44708; the employer did not ensure employees were properly utilizing a fall protection system. The employees were engaged in the installation of roofing material on a commercial roof, the employer was utilizing a warning line system that was not erected around all sides of the project, the employees were actively working on multiple areas of the roof exposed to fall hazards greater than 21 feet above the ground.
