29 CFR  1910.178(a)(4): Modifications or additions which affect capacity and safe operation of powered industrial truck were performed by the employer without the manufacturer's prior written approval:  a)  Throughout facility:  On or about October 16, 2024, and at times prior, the employer exposed employees to fall and blunt force trauma hazards, in that the employer provided a "ULINE" model H-2772 forklift aerial platform for use on a "Linde" model H20C17600 forklift (serial number PKY391L01958) without written approval from the manufacturer of the forklift where employees used the lift and platform to install lighting, perform inventory and hang signs.

29 CFR  1910.178(l)(1)(i):The employer did not ensure that each powered industrial truck operator is competent to operate a powered industrial truck safely, as demonstrated by the successful completion of the training and evaluation specified in this paragraph (l):  a)  Throughout Facility:  On or about October 16, 2024, and at times prior, the employer exposed employees to struck-by hazards, in that the employer did not provide powered industrial vehicle operator training to employees who operate the "Toyota" model 8FGCU32 forklift (serial number 64156), "Linde" model H20C17600 (serial number PKY391L01958) and "Caterpillar" model GC45KS1 forklift (serial number AT87A01307) powered industrial vehicles to move rolls of turf, move pallets or perform maintenance activities in the workplace.
