29 CFR 1910.1026(d)(2)(iii): The employer used the scheduled monitoring option, and monitoring revealed employee exposures to chromium (VI) to be at or above the action level; however, the employer did not perform periodic monitoring at least every six months: On or about October 17, 2024, employees working in the High-Side Assembly Welding Area were exposed to an 8-hour time weighted average (TWA) of 3.0 �g/m3 for Chromium (VI). The exposure level was derived from a sample collected over 453 minutes sampling period with zero exposure assumed for the unsampled period of 27 minutes. The employer failed to perform periodic monitoring at least every six months when employees' exposure to Chromium (VI) was at or above the action level.

29 CFR 1910.1026(k)(1)(i)(A): The employer did not make medical surveillance available for all employees who were or could be occupationally exposed to chromium (VI) at or above the action level for 30 or more days a year: On or about October 17, 2024, employees conducting welding operations on stainless steel in Building 2, High Side Assembly was exposed to an airborne concentration to chromium (VI) at an 8-hour Time Weighted Average (TWA) of 3.0 micrograms per cubic meter (ug/m3) above the action level of 2.5 ug/m3. The exposure was derived from samples collected over a 453-minute sampling period, with zero exposure assume for the period of 27 minutes, sampling conducted on June 20, 2023, and employee was not provided the opportunity for medical surveillance.
