29 CFR 1910.147(c)(4)(i): Procedures were not developed, documented, and utilized for the control of potentially hazardous energy when employees were engaged in activities covered by this section: (a) On or before October 24, 2024, employees were not protected from machinery hazards related to the unexpected energization, startup, or release of stored energy while removing, cleaning, and replacing the spool valve on the bottom of the depositor of the Hinds-Bock twin servo pump filler on line 2 in that the employer did not ensure utilization of specific energy control procedures. (b) On or before October 24, 2024, employees were not protected from machinery hazards related to the unexpected energization, startup, or release of stored energy while removing, cleaning, and replacing the spool valve on the bottom of the depositor of the Hinds-Bock twin servo pump filler on line 3 in that the employer did not ensure utilization of specific energy control procedures.

(b) On October 18, 2024, employees were not protected from any part of their body being caught in the spout spool at the low end of the depositor of the Hinds-Bock twin servo pump filler on line 3 in that its point of operation was not guarded when unclogging during normal operation.
