29 CFR  1926.21(b)(2): The employer did not instruct each employee in the recognition and avoidance of unsafe conditions and the regulations applicable to his/her environment to control or eliminate any hazards or other exposure to illness or injury:  a) Worksite - On or about October 18, 2024, an employee was exposed to electric shock hazards when plugging in an extension cord with a bent prong on the plug end into an electrical outlet (Panel RPW4, Circuit 9, 20 amp, 120v, single phase system).  The employee had not received any instruction or training in the recognition and avoidance of using a damaged extension cord with a bent prong.
