On or about October 22, 2024, power-operated equipment with operating deficiencies were used within the workplace without being taken out of service or repaired. 1) On the left side of the store, a forklift's reverse alarm was not working while an employee was pulling lumber for a customer. 2) On the right side of the store, a Ballymore's closed entry door alarm was not working while the employee was raising themself up approximately 6 feet to retrieve merchandise in the garden department.
