29 CFR  1910.147(c)(4)(i): Procedures were not developed, documented and utilized for the control of potentially hazardous energy when employees were engaged in activities covered by this section:  a) Warehouse floor/ Packaging Department  - On or about 10/18/2024, the employer exposed employees to amputation hazards and electrical shock hazards in that, the employer failed to ensure the hazardous energy control lockout/tagout procedures were developed, documented, and implemented for the employees during servicing and maintenance of equipment, such as but not limited to clearing jams from the Fam Cutter.

29 CFR  1910.147(c)(7)(i)(A): Authorized employee(s) did not receive training in the recognition of applicable hazardous energy sources, the type and magnitude of the energy available in the workplace, and the methods and means necessary for energy isolation.   a) Warehouse floor/ Packaging  Department - On or about 10/18/2024, the employer exposed employees to amputation hazards and electrical shock hazards in that, the employer failed to ensure  authorized employees performing servicing and maintenance of equipment were provided with training while engaged in activities for clearing jams from the Fam Cutter.
