29 CFR  1926.20(b)(2): The employer did not initiate and maintain programs which provided for frequent and regular inspections of the job site, materials and equipment to be made by a competent person(s):  a) First floor - On or about October 23, 2024 and at times prior thereto, the employer did not perform any inspection of the worksite prior to employees performing on site work.  An employee was injured after falling through an unprotected floor hole that was present on site.

An employee was injured after falling through an unprotected floor hole that was present on site.
