29 CFR  1926.602(c)(1)(vi): Industrial truck(s) in use did not meet the applicable requirements of design, construction, stability, inspection, testing, maintenance, and operation, as defined in American National Standards Institute B56.1-1969, Safety Standards for Powered Industrial Trucks.  a) At  the site located at 7156 SR-99 Brunswick, GA 31525, on or about November 5, 2024, the employer exposed employees to struck-by and crushing hazards in that, the employer did not ensure the path of travel for an operator of a JCB Loadall Telescopic Forklift, model 507.42 F, was free of loose objects and debris.  b)  At  the site located at 7156 SR-99 Brunswick, GA 31525, on or about November 5, 2024, the employer exposed employees to struck-by and crushing hazards in that, the employer allowed an operator of a JCB Loadall Telescopic Forklift, model 507.42 F, to transport an unstable load of two bundles of #3 rebar.

For each employee engaged in a task identified on Table 1, the employer did not fully and properly implement the engineering controls, work practices, and respiratory protection specified for the task on Table 1.  a) At the site located at 7156 SR-99 Brunswick, GA 31525 on or about November 5, 2024, the employer exposed employees to respiratory and inhalation hazards in that dust collection systems or wet methods were not provided to employees while drilling into concrete to install rebar and dowels.
