29 CFR 1910.1200(e)(1): Employer had not developed or implemented a written hazard communication program that included the requirements outlined in 29 CFR 1910.1200(e)(1)(i) and (e)(1)(ii):  A written HAZCOM program was not developed for employees required to work with hazardous chemicals such as Denatured Alcohol, Krud Cleaner, Brakleen, paint, and lacquer thinner, that were use at the jobsite. Employees were exposed to chemical hazards.

29 CFR 1915.36(a)(1): Smoking, open flames, arcs, or spark producing equipment were not prohibited in areas where liquid solvents, paint and preservative coating removers, paints or vehicles capable of producing a flammable atmosphere under the conditions of use, were used.  Nakia Engine Compartment: The vacuum used to remove sludge build up from cleaning the engine compartment with volatile flammable solvents was not rated for use in flammable environments. Employees were exposed to explosion and fire hazards.

29 CFR 1915.36(a)(2): Ventilation was not provided in sufficient quantities to keep the concentration of vapors below 10 percent of their lower explosive limit in areas where liquid solvents, paints, or preservative removers were capable of producing a flammable atmosphere.  Nakia Engine Compartment: Ventilation was not provided while an employee used denatured alcohol and Brakleen to clean the compartment. An explosion and subsequent fire occurred when the employee switched on a shopvac to remove bulk debris. The employee was exposed to explosion and fire hazards.

29 CFR 1910.134(g)(1)(i)(A): Respirators with tight-fitting facepieces were worn by employees who had facial hair that came between the sealing surface of the facepiece and the face or that interfered with valve function:  The employer did not prohibit respirator use by an employees who had facial hair that came between the face and the sealing surface of the respirator facepiece. Employee were required to wear a half-face elastomeric respirators while performing painting and sanding activities. Employees were exposed to respiratory hazards.   PDF, INC was previously cited for a repeat violation of this Occupational Safety and Health Standard 29 CFR 1910.134(g)(1)(i)(A), which was contained in OSHA inspection number 1502252, citation number 2, item 1 and was affirmed as a final order on February 16, 2021, with respect to a workplace located at 24 Sand Island Access Rd. #14
