On or about November 08, 2024, the employer did not ensure the direct supervision of a trainee operating the Crown, Model: PE 4500-60, Serial Number: 10042786, Electric Pallet Jack was maintained. The employee was caught-in-between a stationary pallet and the electric pallet jack as a result.
