29 CFR  1928.57(a)(6)(iii):Instruction was not provided to stop engine, disconnect the power source, and wait for all machine movement to stop before servicing, adjusting, cleaning, or unclogging the equipment:  On or about 11-22-24, employees engaged in chicken catching operations located at 3345 I Road, Rising City, Nebraska 68658, were exposed to amputation hazards in that the employer failed to provide instruction to an employee to wait until the engine was stopped, all power sources were isolated, before attempting to adjust a malfunctioning conveyor belt on the Apollo Generation 2 machine.

29 CFR  1928.57(a)(11):Where a moving machinery part presented a hazard during servicing or maintenance, the engine was not stopped, the power source was not disconnected, and all machine movement was not stopped before servicing or maintenance was performed, except where the Employer could establish the following in 1928.57(a)(11)(i-iii):  On or about 11-22-24, employees engaged in chicken catching operations located at 3345 I Road, Rising City, Nebraska 68658, were exposed to amputation hazards in that the employer failed to ensure that the Apollo Generation 2 Chicken catching Machine was fully stopped, with the power source disconnected prior to adjusting the conveyor belt on the front side of the machine.
