29 CFR 1910.212(a)(3)(ii):  Point of operation guards were not designed and constructed as to prevent the operator from having any part of their body in the danger zone during the operating cycle.  a.  On or about November 14, 2024, in the production area, employees were required to operate the Line 12 AMF pizza bread dough extruder, Model ADD, Serial #95192 without the point of operation being completely guarded.  Employees were thereby exposed to point of operation hazards when an employee was able to reach underneath the guard to the port where knife blades performed a shearing action during normal production.
