The employer failed to barricade the drop zone area of the tree being felled and prohibit employees from returning to the area until it was safe to return.  An employee was in the drop zone holding a flashlight to assist the saw operator when a limb fell striking the employee.

The employer failed to conduct a job briefing before the start of each job to all affected workers. The crew leader/foreman/arborist in charge failed to instruct employees on the hazards associated with the job, work procedures involved, special precautions for performing work in the dark, and personal protective equipment requirements to protect all employees from struck by hazards from falling limbs and debris while felling trees as part of electrical line clearing operations.
