29 CFR  1926.451(b)(7):On scaffolds where platforms are overlapped to create a long platform, the overlap shall occur only over supports, and shall not be less than 12 inches (30 cm) unless the platforms are nailed together or otherwise restrained to prevent movement.  a) On December 30, 2024, employees working from frame scaffolding were exposed to fall hazards of up to approximately 13.2 feet when working on an overlapped platform.   In accordance with 29 CFR 1903.19(c), abatement certification is required for this violation (using the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET).

29 CFR  1926.451(c)(1)(ii): Supported scaffolds with a height to base width ratio of more than four to one (4:1) were not restrained from tipping by installing guys, ties, and braces at the closest horizontal member to the 4:1 height and were not installed at each end of the scaffold:   a) On December 30 , 2024, employees were exposed to fall hazards while working on frame scaffolding exceeding a 4:1 height to base width ratio which was not restrained on both ends from tipping.  In accordance with 29 CFR 1903.19(c), abatement certification is required for this violation (using the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET).
