OSH ACT of 1970 Section (5)(a)(1): The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that they were exposed to struck-by and crushing hazards of collapsing storage rack systems due to damaged components and multiple missing bolts to secure racks to the floor.   Location: Warehouse, 21 Ethan Allen Road, Newtown, CT.  1. On or about January 2, 2025, and times prior to, employees were exposed to struck-by and crushing hazards when conducting warehouse tasks near pallet rack storage systems that were damaged due to forklift impacts. The storage racking systems had multiple damaged columns due to forklift impacts.  2. On or about January 2, 2025, and times prior to, the employer did not ensure that all rack columns were anchored to the floor with anchor bolts adequate to resist all applicable forces.
