29 CFR  1910.180(b)(3): The employer did not ensure only designated personnel were permitted to operate a crane covered by this section:  a) At 1785 NE 35th Street, Ocala, Florida 34479; On or about 01-02-2025, the employer did not ensure the designated personnel operating a National Crane 900A, 52000 lb. truck mounted crane during tree cutting operations had the knowledge, training, and/or experience to safely operate the crane, exposing employees to roll-over, crush-by, and struck-by hazards.

29 CFR  1910.180(h)(3)(ix)(c): Wood blocks used to support outriggers were not of sufficient width and length to prevent shifting or toppling under load:  a) At 1785 NE 35th Street, Ocala, Florida 34479; On or about 01-02-2025, the employer exposed employees to rollover, struck-by and crushed-by hazards, in that; the wood cribbing used to support a truck-mounted crane's outriggers were not of sufficient width and/or length to prevent shifting or toppling under load, as employees utilized the crane during tree-felling operations.

29 CFR  1910.180(d)(6): Monthly, dated and signed inspection records and reports for truck crane(s) were not provided for critical items such as (but not limited to) brakes, crane hooks and ropes:  a) At 1785 NE 35th Street, Ocala, Florida 34479; On or about 01-02-2025, the employer did not have available to OSHA, the certification records of the monthly inspections, that included the date of inspection, the signature of the person who performed the inspection and the serial number, or other identifier, for a truck-mounted crane.
