29 CFR 1910.1200(h)(1): Employees were not provided effective information and training on hazardous chemicals in their work area at the time of their initial assignment and whenever a new hazard that the employees had not been previously trained about was introduced into their work area: On or about January 17, 2025, the employer did not provide employees with effective information and training that identified the potential hazards employees were exposed to while using hazardous chemicals such as but not limited to Hand Sanitizer, Drug Buster, Clinging Bowl & Porcelain Tile Cleaner, Hydrogen Peroxide Topical Solution, Engineered Fuel & Oil, and Paint Thinner. All provisions of 29 CFR 1910.1200(h)(1) through (h)(3) must be provided to employees as part of hazard communication information and training. Key provisions include, but are not limited to the following: 1) Details and requirements of the Hazard Communication standard 2) Location and availability of SDS, chemical list and written program 3) Operations where hazardous chemicals are present 4) Physical, health and other hazardous of chemicals in the work area 5) Mean of protection from chemical hazards 6) Explanation of labels, labeling systems and SDS

29 CFR 1910.36(a)(3): An opening into an exit was not protected by a self-closing fire door that remains closed or automatically closes in an emergency upon the sounding of a fire alarm or employee alarm system. Each fire door, including its frame and hardware, must be listed or approved by a nationally recognized testing laboratory. Section 1910.155(c)(3)(iv)(A) of this part defines "listed" and � 1910.7of this part defines a "nationally recognized testing laboratory." On or about January 17, 2025, the employer failed to ensure the fire rated door on the south end of West Hallway markings were visible to identify the certifying authority or fire rating of the door.
