29 CFR 1910.178(a)(5):The powered industrial truck was equipped with front-end attachments other than factory installed attachments, however the employer did not request that the truck be marked to identify the attachments and show the approximate weight of the truck and attachment combination at maximum elevation with load laterally centered: Employee(s) were exposed to struck-by hazards. This was observed on or about January 22, 2025, at 5120 26th Street, Columbus, Nebraska. Employees were exposed to struck-by injuries in that the Toyota Forklift used extended forks; however, it was not marked in accordance with this sub part.

This was observed on or about January 22, 2025, at 5120 26th Street, Columbus, Nebraska. A flexible cord on a drop box by the Mazak #2 did not have strain relief.
