29 CFR 1910.147(c)(6)(i): The employer did not conduct a periodic inspection of the energy control procedure at least annually to ensure that the procedure and the requirement of this standard were being followed.  On or about January 23, 2025, at Tower Metal Works, Inc. located at 29273 Windy Hill Rd. Maple Hill, Kansas, employees were exposed to struck-by and crushing hazards in that the employer did not conduct periodic review of the energy control procedures for the HEM saw, Cincinnati Shear and the GH 1303 press break.
