29 CFR 1910.212(a)(1): One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks: (a) On or about January 24, 2025, the employer did not ensure that the Warner & Swasey 3A Lathe (model no. M3500, serial no. 2018182) rotating chuck was properly guarded to protect employees from entanglement hazards. (b) On or about January 24, 2025, the employer did not ensure that the HYD-MECH Horizontal Band Saw (model no. M-16A, serial no. 50606388) had unused portions of the saw blade guarded which exposed employees to amputation hazards. (c) On or about January 24, 2025, the employer did not ensure that the HYD-MECH Horizontal Band Saw (model no. H-28, serial no. D0998002D) had a damaged guard with the blade exposed which exposed employees to amputation hazards. (d) On or about January 24, 2025, the employer did not ensure that the HYD-MECH Horizontal Band Saw (model no. H-12A, serial no. A0395285) had unused portions of the saw blade guarded which exposed employees to amputation hazards. (e) On or about January 24, 2025, the employer did not ensure that the HYD-MECH Horizontal Band Saw (model no. H-22, serial no. UNKNOWN) had unused portions of the saw blade guarded which exposed employees to amputation hazards.
