29 CFR 1910.147(f)(3)(ii)(D): Each authorized employee did not affix a personal lockout or tagout device before working on the machine or equipment:  a) Production - On February 6, 2025, the employer did not ensure that each employee performing work on machines or equipment, including sanitation and when using scrapers to remove dough on Line 30 and 31, affixed a personal lockout or tagout device.  In accordance with 29 CFR 1903.19(c), abatement certification is required for this violation (using the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET).

29 CFR 1910.212(a)(1): One or more methods of machine guarding were not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks:    a) Production: Line 30 - On or about January 25, 2025, the employer did not ensure that employees were adequately protected from rotating parts and ingoing nip points on equipment, such as the sheeter roller on the Bombay Gate.   No abatement certification or documentation is required for this item.
